Class Register Form Once you have registered for Values III class, you must fill this Register Form to receive weekly updates, announcements, online-tests, and other important information. Please fill this form at the beginning of the School Year.
Message Board You can post your message on this Message Board which is made just for you! You need to signup to use this Message Board features.

SLPT Sign-up Form

 You are requested to Sign-up for SLPT conference using this Form. Using this form allows the teacher and yourself to keep track of your appointment. Fill this form one week before the SLPT conference date. SLPT conference is held twice a year: 2nd quarter and 4th quarter.
Request a Re-test Form
 If you have failed or did poor in a test, please fill this form to request the teacher to create another test just for you. Note: in a re-test, you can earn maximum of 87% only. You must however have a good reason with proper excuse forms/letters to have your request approved.
Enquire Grade Form
If you have a doubt about your grade, fill this form and indicate to the teacher to send you your detail grade through email.
Contact the Teacher Form If you need to contact the teacher or wish to send a feedback to the teacher, fill this form without disclosing yourself (remain anynomous).
